With the North Texas Institute of Neurology, Rheumatology & Headache in Plano, TX, Dr. Sorin has designed a practice that he prides himself on having an old-school feel. He is committed to spending time with his patients, getting to know them, and gaining insight into their ailments.
Dr. Sorin devotes a great deal of time to managing and treating all forms of headache. "We do our best to allow patients access when they need it," he says. Dr. Sorin performs most of his own procedures, which often include various nerve blocks, BOTOX® for migraines, Dysport® for dystonic neck pain, and fluoroscopic-guided lumbar puncture.
Dr. Sorin diligently tries to understand the triggers of his patients' headaches and takes a hands-on approach to treating the underlying causes. He also runs a full neurodiagnostic center, including nerve conduction studies, EEG, home video EEG monitoring, and neurocognitive screening for attention deficit disorder, post-concussion syndrome, and memory.
Dr. Sorin ranks among the top neurotoxin injectors in the country (BOTOX, XEOMIN®, Dysport) for a myriad of injections, which include migraine, cervical dystonia, atypical facial pain, trigeminal neuralgia, oromandibular dystonia (TMJ), hemifacial spasm, and acquired dystonic reaction associated with previous injury or surgery, just to name a few. Many patients have a combination of these ailments and his expertise allows him to develop the best treatment program for each one.
Dr. Sorin has dedicated much effort toward public awareness of traumatic brain injury and concussions. He has been spearheading an effort to screen athletes to maintain a baseline cognitive function in case of future head injuries.

Dr. Bontia completed medical school at the University of the Philippines. She did her neurology residency at the North Shore-Long Island Jewish Hospital in Manhasset, NY where she also served as chief resident. She then went on to the University of North Carolina at Chapel Hill and completed a fellowship in neuromuscular medicine.
Dr. Bontia treats all neurological patients but has a special interest in patients with neuromuscular diseases (i.e., peripheral neuropathy, myasthenia gravis, myopathy, etc.) and headaches. She performs neurotoxin injections for various conditions, including spasticity, cervical dystonia, and chronic migraines. Dr. Bontia's board certifications are as follows:
Dr. Bontia has been practicing neurology since 2006, and she uses experience, skillful analysis, compassion, and advanced treatment techniques and technologies when evaluating her patients. She takes time to listen and treats each patient as a partner in his or her own neurological care. Each patient gets individualized treatment, and Dr. Bontia does not hesitate to use various treatment interventions (medicines, injections, massage, acupuncture, and physical therapy) to help her patients heal.
Dr. Timm is a headache physician with a strong background in primary care. Dr. Timm has been trained in all aspects of headache medicine, including migraine, cervicogenic headache, facial pain, and medication overuse.

Dr. Annette Okai is fellowship trained in multiple sclerosis. She is the director of neuroimmunology and multiple sclerosis research at North Texas Institute of Neurology, Rheumatology & Headache in Plano, TX, and a clinical associate professor of neurology at Texas Tech University Health Sciences Center. Dr. Okai is a diplomate of the American Board of Psychiatry and Neurology.
In addition to her clinical practice, she is actively engaged in multiple clinical trials in multiple sclerosis and has been published in several neurological journals.
She is a graduate of MCP Hahnemann School of Medicine (now Drexel School of Medicine) in Philadelphia. She completed her neurology residency at Thomas Jefferson University Hospital. Dr. Okai was the recipient of the National MS Society fellowship award and a fellowship grant from the National Institutes of Health. She completed a two-year fellowship in clinical neuroimmunology at Thomas Jefferson University Hospital and a graduate certificate in public health.
Dr. Varuni Rao graduated from the New York College of Osteopathic Medicine in Old Westbury, NY. She completed her internship and residency in internal medicine at New York Medical College at Westchester Medical Center in Valhalla, NY where she was named chief resident and awarded best resident in ambulatory clinic/continuity of care.
She completed her fellowship in rheumatology at New York Medical College at Westchester Medical Center and is board certified in rheumatology.
Dr. Rao is a rheumatologist who specializes in managing various autoimmune disorders, such as rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis, inflammatory bowel disease-associated arthritis, crystal-induced arthropathies (gout and pseudogout), lupus, myositis, and other musculoskeletal disorders.

Sara Freeman is a licensed certified physician assistant. Born and raised in New Orleans, LA, she earned her undergraduate degree from Louisiana State University and a Master of Medical Science in physician assistant studies at Our Lady of the Lake College in Baton Rouge, LA.
She began her career in general neurology, treating chronic migraine headaches, cerebrovascular disease, neuropathy, radiculopathy, epilepsy, and many other general neurological conditions. She then specialized in sleep medicine, helping manage sleep-related breathing disorders, insomnia, narcolepsy, and restless legs syndrome.
Sara is an empathetic and motivated medical professional. She values the ability to help reduce acute and chronic headache symptoms with conservative pharmacological and injection treatments. Sara supervises infusion treatments, performs trigger point injections and nerve blocks, and administers BOTOX for chronic migraine headaches. She particularly enjoys managing sleep disorders, which can be a significant contributing factor to headaches.
Sara is a member of the American Academy of Physician Assistants and the Texas Academy of Physician Assistants.
Dena Miller is a board-certified nurse practitioner. She has a particular interest in multiple sclerosis and headaches and has certifications as both a multiple sclerosis-certified specialist through the Consortium of Multiple Sclerosis Centers and certified in headache medicine through the National Headache Foundation. She holds a Bachelor of Science in Nursing from Lubbock Christian University and a Master of Science in Nursing from Texas Tech University Health Sciences Center. She is board certified as a nurse practitioner through the ANCC and has been practicing in neurology for ten years with experience in all aspects of neurology. She has also been a sub-investigator in clinical research protocols, examining the safety, efficacy, and tolerability of treatments related to multiple sclerosis. Ms. Miller is a member of many professional organizations, including the American Academy of Neurology, the Texas Neurological Society, the National Headache Foundation, the Southern Headache Society, and the Consortium of Multiple Sclerosis, and is involved with the MS Society. She is also an active speaker in the fields of multiple sclerosis and headache.
Kylie is a licensed certified physician assistant. Born and raised in Atlanta, GA, she attended Georgia State University and graduated with a Bachelor of Science in Neuroscience. She then attended Wingate University in North Carolina and earned her master's degree in physician assistant studies. She is board certified through the National Commission on Certification of Physician Assistants.
She began her career with the North Texas Institute of Neurology, Rheumatology & Headache where she held roles in headache management, rheumatology, and the Acute Center for Headache Emergency (A.C.H.E.) department. She values a combination of lifestyle modification, pharmaceutical intervention, and medical injections to manage headaches.
When she is not in the clinic, Kylie enjoys weight training, live music, and spending quality time with friends and family.
Annie is from Louisiana where she earned a Bachelor of Science in Biology from Louisiana Tech University. She then attended Mississippi College where she received her Master of Science in Medicine in physician assistant studies in 2022. Annie is board certified through the National Commission on Certification of Physician Assistants (NCCPA). She began her medical career working at the North Texas Institute of Neurology, Rheumatology & Headache in headache medicine, as well as working in the Acute Center for Headache Emergency (A.C.H.E.) department.
When Annie is not in the office, she enjoys spending time with her family and friends, participating in outdoor activities, traveling, and listening to live music. She is passionate about providing the highest quality of patient care and strives to build trusting relationships with all her patients.
Tony is a licensed certified physician assistant. He attended the University of Texas Southwestern Medical Center where he earned his Master of Physician Assistant Studies in 2023. Tony is board certified through the National Commission on Certification of Physician Assistants (NCCPA).
He began his career with the North Texas Institute of Neurology, Rheumatology & Headache where he held roles in general neurology, headache management, and weight loss and worked in the Acute Center for Headache Emergency (A.C.H.E.) department.
Tony is a member of the American Academy of Neurology (AAN) and the Obesity Medicine Association (OMA). He is working toward his certification in headache medicine by the National Headache Foundation and his certification in obesity medicine by the OMA.
Paige Wolfe is a board-certified nurse practitioner through the American Association of Nurse Practitioners. She received her Bachelor of Science in Nursing from St. Luke's College of Health Sciences in Kansas City, MO, and went on to receive her doctorate in nursing practice from Maryville University in St. Louis, MO. After working as a registered nurse for seven years, she began her career as a nurse practitioner at the North Texas Institute of Neurology, Rheumatology & Headache, treating chronic migraines and headaches, as well as working in the Acute Center for Headache Emergency (A.C.H.E.) department. Paige values a combination of lifestyle modification and pharmacological intervention to manage chronic migraines and headaches.
